SMS Opt-In Proof
Forevify uses a staff-mediated opt-in workflow. Activity Directors at partnered senior-living facilities add residents to Forevify through the interface below and capture explicit SMS consent for each resident.
How opt-in works
- A resident (or their authorized representative) verbally consents to receive Forevify SMS messages as part of their onboarding at a partnered facility.
- The facility’s Activity Director opens the “+ Add Resident” form in the Forevify portal (shown below), enters the resident’s name and mobile number, and reads the SMS consent disclosure.
- The AD checks the SMS consent box only after confirming that consent has been given. The checkbox is unchecked by default and must be affirmatively clicked.
- On save, Forevify sends a one-time welcome SMS that re-states the message purpose, frequency, and STOP/HELP keywords. The resident can opt out at any time.

Message Types & Frequency
- Welcome SMS — one-time on opt-in
- Birthday and holiday song invitations — from facility Activity Director, typically 1–3 per week per resident
- Song-ready notifications — when a song the resident helped create is delivered
- Reply-to-family notes — when a family member replies to a song
Total message volume varies per resident, but averages approximately 4–8 messages per month. Message and data rates may apply. Recipients can reply STOP at any time to unsubscribe, or HELP for support contact info.
